- Medical Director Leads the Interdisciplinary team in the development of the plan of care. Provides consultation to maximize quality of life.
- Registered Nurses Coordinates the care with the attending physician and addresses the symptoms or pain management. Render direct patient care. Formulates plan of care.
- Medical Social Worker Provides intensified psychosocial and emotional support to the family and patient throughout their journey.
- Spiritual Counselor Provides Spiritual care services that are consistent with the patient & family’s beliefs. Understands & supports your grief and loss.
- Certified Hospice Aide Provides direct personal care and comfort measures to patients as determined by the plan of care. Reports the identified need to the assigned RN Case Manager.
- Volunteers Provides Companionship and support to patients and families through different activities.
- Physical Therapy Is part of hospice medicare benefit that is usually untapped. Physical therapy maximizes and helps maintain the ADLs of our loved ones keeping them dignified.
- 24/7 triage On-Call Skilled Nurse Kind Heart provides 24/7 - 365 days of support at anytime of the day. A live nurse will always assist you and guide you through your questions and challenges. We will come at anytime to where the patient is at.
